About the Town of Lake Hamilton Veteran’s Wall

The Town of Lake Hamilton Veteran’s Wall honors military personnel who have lived in the town. The wall was dedicated on July 2, 2015.  This wall was designed to display plaques as a tribute to both hometown military veterans as well as those who paid the ultimate sacrifice.  Lake Hamilton recognizes and respects the enormous sacrifices made by those serving in our armed services.

To be represented on the veteran’s wall, hometown veterans must meet the requirements listed below. The plaque will list the name, rank, years of service, and military branch. If the veteran is killed in action while serving, a star will be placed next to the dates of service. (Purple Heart Recipient)

Requirements to have a Plaque placed on the Wall:

  • Veteran must have been born and/or resided in the Town of Lake Hamilton for a minimum of 5 years.
  • Must provide proof of residency at some point during veteran’s life (Property records, tax statement, historical document, affidavit of friend, relative or former neighbor). If deceased, please provide (obituary or death certificate).
  • Must provide proof of service (see below)

Accepted Documents that Prove Military Service;

  • Copy of Department of Defense (DD Form 214) Honorable Discharge
  • VA Issued ID Card for Health Care
  • Veterans ID Card
  • Veterans Designation on Drivers License or State Veterans ID Card 
  • Additional Ways to Prove Military Service:
  • The DD Form 215, 256, NGB Form 22 and similar forms are official proof of service documents. These are the forms usually required for applying for veterans benefits through the Department of Veterans Affairs and other official benefits. 

Application for Lake Hamilton Veteran’s Wall Plaques

  • Applications may be picked up at the Lake Hamilton Town Hall, Clerks Office.
  • Cost of plaque shall be determined at time of order to includes plaque (w/tax), installation, and lifetime maintenance cost.
  • Veteran / Family member shall be responsible for the purchase of the plaque at time of order.
